Case Manager
Salary: $22.50 - $26.50 per hour
Schedule: Tuesday-Saturday | Shifts: 7:00 am - 3:30 pm (1 opening) & 3:00 pm - 11:30 pm (2 openings)
About the Position
We are seeking a compassionate, driven Case Manager to join our team at the Yale Navigation Center. This essential role involves meeting with clients to assess their needs, provide guidance, and develop plans to support their journey toward stable housing and improved well-being.
As a Case Manager, you’ll work on-site to establish positive client relationships, assist with individualized support plans, and connect clients to resources. This position requires a valid driver’s license, reliable transportation, and insurance, as the role will eventually involve meeting clients in the community.
Key Responsibilities
- Conduct client meetings, assessing needs and developing personalized support plans.
- Connect clients with resources, services, and community programs to aid in housing and health goals.
- Document client interactions and progress, ensuring confidentiality and professionalism.
- Collaborate effectively with colleagues and community stakeholders to support client needs and program goals.

Preferred Qualifications
- 2 years of experience in healthcare, case management, homeless services, or working with vulnerable populations.
- Bilingual skills are highly valued (any language besides Spanish is a plus).
Requirements
- Valid driver’s license, reliable transportation, and insurance.
- CPR/First Aid certification (or willingness to obtain).
- Ability to pass a background screening, drug test, and tuberculosis test.
